As much fun as your child will likely have in our waiting room, we know that it can be tough keeping an eye on your child while filling out numerous patient forms! To help speed up the check-in process, please take a few minutes fill out our new patient forms before your first appointment:

We accept dental insurance, including most PPO plans and Florida Healthy Kids / Medicaid plans. We are always happy to file your claim for you as a courtesy and encourage you to call if you have questions. We recommend that you contact our office prior to your appointment to provide us with insurance information for verification of benefits. We accept all major credit cards and offer financing programs through Care Credit (https://www.carecredit.com/) for those who qualify. Payment for treatment is due at the time of service. For more details about insurance and/or our accepted forms of payment, including our in-house financing and payment plans, please contact our office to speak with a member of our team.
